In case of Domestic Shipment:
Declaration letter of the insured/insurer regarding the damaged commodity
Original or photocopy of the policy
Original or copy of commodity invoice
Original or copy of delivery note
Copy of freight invoice
Traffic accident report / alcohol examination (if there is a traffic accident)
Reports issued by the official authorities regarding the damage, if any
Damage assessment report (including lorry/truck number plate and driver's signature)
- Damage Notification letter (Written Warning) sent to the carrier company from the notary public by the insured/insurer
Signature circular
Photographs of damaged commodities
Bank details of the insured
IBAN details
- Other (additional) documents deemed necessary by the insurer depending on the nature of the damage

In case of International Shipment:
Declaration letter of the insured/insurer regarding the commodity
Original or photocopy of the policy
Original or copy of commodity invoice
Bill of lading in maritime transport, freight bill or CMR in international road transport, airway bill in airway, railway bill in railway
- Customs entry declaration (in case of import)
Customs exit declaration (in case of export)
Damage assessment report (including the carrier's signature)

Packing list
Contract of carriage, freight (transport fee) invoice
Damage notification letter (Written Warning) sent to the carrier company from the notary public by the insured/insurer

In case of Domestic Carrier Liability Insurance Damages:
Declaration letter of the insured/insurer regarding the damaged commodity,

Traffic accident report / alcohol examination (if there is a traffic accident)
Reports issued by the official authorities regarding the damage, if any

Documents for the transport vehicle
Invoice and letter certifying that the owner of the goods holds the carrier responsible for the damage within the framework of its legal liability

A copy of the transport commodity insurance for the commodity, if any, or a declaration letter from the owner stating that there is no commodity policy
Bank details of the insured
